STEP 1	Call and speak with a knowledgeable Progenix HEALTH specialist. They are available to answer questions regarding program options, completing required documents, and assist with the simple process to get you started today.
STEP 2	Schedule an appointment with a Progenix HEALTH Physician. Your specialist will schedule an appointment for you to meet with one of our health management physicians. At which time you’ll receive a comprehensive medical evaluation, consultation, blood draw, and physical. We have a national network of physicians and labs allowing us to easily locate one that’s close and convenient to you.
STEP 3	Complete and submit the online medical history form: (All Information you provide is confidential) When completing this form, please take your time and answer all questions honestly, and as accurately as possible. This information will play a significant role in what ultimately determines your therapy. Once completed please click on the submit button at the bottom of the page. A window will appear confirming you have successfully submitted the completed form. This information will be submitted directly to us and a copy will be forwarded to the email address you provided. Upon receipt of the email you have successfully completed and submitted your on-line Medical History form. You may print and/or save a copy of the attached document for your records. Please remember to sign, date, and fax the “CLIENT AGREEMENT AND RELEASE” section, along with a copy of your drivers license, to (561) 429-8382 (It is required by law that you provide us with a copy of your Driver's License for identification purposes)

RESULT	Once we’ve received your completed information its presented to a Progenix HEALTH physician and taken under review. Based on the physicians professional medical opinion, they will determine what they feel would be the most effective program, specifically designed for your particular needs. Once this decision has been established, you will then be contacted to discuss your new therapy options.

Completing the required forms does not automatically qualify you for Hormone Replacement Therapy. Our physician must determine, based on their professional medical opinion, and after review of your medical history, physical examination, lab results, and evaluation that you demonstrate a medical need for treatment.
